Service Description: Provides treatment, care, and community supervision to people with mental illness instead of  a longer stay in hospital or as an alternative to hospitalization * helps identify what the client and their support system must to do to help them stay well and out of hospital


Application: Referrals come from the CMHA-WECB CTO coordinator at Windsor Regional Hospital – Ouellette campus
Eligibility / Target Population: People 16 years and older who have a history of repeated hospitalizations and who meet the committal criteria for the completion of an application by a physician for a psychiatric assessment under the Mental Health Act * involuntary psychiatric patients who accept a treatment/supervision plan as a condition of their release from psychiatric facility to the community
