16-week program that assists people diagnosed with moderate to severe Chronic Obstructive Pulmonary Disease (COPD) and other respiratory conditions * educational sessions taking place three times per week * operated by an inter-professional team comprised of disciplines including physiotherapy, respiratory therapy, pharmacy, dietary, clinical ethics, speech language pathology and occupational therapy
