Service Description:

Supportive Care and Bereavement Services are dedicated to providing compassionate care and improving the quality of life for individuals (including caregivers) living with a life threatening or life altering, chronic, terminal illness, in the Chatham-Kent area. Our services can be provided at home, in a hospital or long-term care setting, and in the community
Bereavement support is offered to those 18 and up in an individual and group setting
