Service Description: 181-bed long-term care home * residential care for private, semi-private and basic rooms * 24-hour nursing and personal care * access to a doctor and other health professionals * dementia/Alzheimer unit

LEVEL OF CARE:
Light Care: Can manage most activities of daily living, may require cueing or supervision; requires assistance with provision of basic needs, i.e. medications, nutrition, hygiene, housekeeping, mobility, etc.
Medium Care: May be able to manage some activities but requires assistance and supervision with activities of daily living; may require cueing or supervision; requires assistance with provision of basic needs, i.e. medications, nutrition, hygiene, housekeeping, mobility, etc.
Heavy Care: Requires total care with most or all activities of daily living, with complex care nursing needs and/or behavioral management difficulties.

SPECIAL UNITS: Two secured area for dementia care.   (Convalescent ON HOLD - beds are currently designated for LTC)10 Bed Convalescent Care program with a maximum stay of 90 days in a calendar year to assist residents who need strengthening and physiotherapy so that they may return to their homes.

SHORT STAY BED TYPE: I respite care bed

STRUCTURE TYPE: Multi-level

FACILITY SECURITY TYPE: Magnetic locks on all doors, security cameras in place, secured dementia unit. Alarms on all exterior doors accessed by residents to unsecure outside areas. Secure patio area

RESIDENT CARE SERVICES: Oxygen (concentrator, liquid), Inhalation therapy, Suctioning, Dressings (aseptic, sterile), Decubitus care, Removal of skin closures, Ostomy care, Catheter care (indwelling, intermittent, irrigation), Palliative care, Intermittent subcutaneous injection, Medications (IM Therapy), Tube feeding, Physiotherapy, music therapy, IV antibiotic Therapy

FACILITY INFIRMARY: NO

FACILITY LIFESTYLE - PERMISSIONS: Alcohol use - as per physician's orders, assessment completed to determine safety of unsupervised smoking, smoking outside only

FURNITURE POLICY: May bring in own furniture (exception is beds and upholstered furniture from home), as long as it does not impede nursing care and meets fire hazard conditions (i.e. television, dresser)

MEAL PLAN / SERVICE TYPES: Common dining room on main floor, as well as dining space on each floor

PHYSICIAN VISIT FREQUENCY: As required

RESTRAINT METHOD: Least restraint policy

PET POLICY: Family pets can visit providing proper documents for the pet
